Your Notice of Admission (NoA) will be sent via email by the third week of June 2025 by the
Admission Section, Office of the University Registrar (OUR), UP Los Baños with the registration
schedule and instructions for First Semester 2025-2026.

For the submission of credentials to the OUR on 21-25 July 2025, kindly prepare the following
entrance credentials for submission on the designated schedule indicated in your NoA:

• UPCAT Acceptance Confirmation Notice (ACN) – Downloaded from the UPCA portal
• UPLB Notice of Admission (NOA) – One page printout of the emailed notice
• Form 138 (First and Second Semester Grade 12 Report Card) – Original Copy
• Form 137 (Senior High School Transcript of Records) – Original Copy
  The link to the letter request for the high school will be sent with the UPLB Notice of Admission
• PSA Birth Certificate (or NSO if the PSA is not yet available)- 2 photocopies with original copy to be presented for verification
• Recent and identical ID pictures – 1×1 with white background and name labelled at the bottom, 3 copies
• Accomplished Student Directory Form – Download copy here.
• Medical Clearance issued by the University Health Service,
  
  UPLB Additional for non-Filipino citizens only:
• Passport pages showing name, photo, date and place of birth – Photocopy
• Study Permit issued by the International Student Relations Office, OVCSA, UPLB

Kindly take note of the tentative dates for registration matters as follows:

24 June 2024 (T). Deadline to accomplish the New Undergraduate Confirmation of Enrollment
Google Form for First Semester 2025-2026.

07 July 2025 (M) to 18 July 2025 (F). Pre-Enrollment Physical/Medical Examination (PPE) for
new first years. Please proceed directly to the UPLB University Health Service on your assigned
date (will be indicated in the registration instructions).

21 July 2025 (M) to 25 July 2025 (F). Advance registration (submission of hard copies of
entrance credentials) for new first years. Please proceed to Windows 2 and 3 of the Office of the
University Registrar, G/F CAS Annex I Building, Pedro R. Sandoval Avenue on the assigned
registration date for your degree program (will be indicated in the registration instructions).

Only those who have accomplished the Undergraduate Confirmation of Enrollment Google
Form 2025-2026 and submitted their entrance credentials will be issued the UP Mail account and the Academic Management Information System (AMIS) credentials.

First week of August 2025. Access your AMIS account to view and download the schedule of
classes for First Semester 2025-2026.
