Please wait for the email from the Office of the University Registrar (OUR), UP Los Baños for
your UPLB Notice of Admission, registration schedule and instructions for First Semester 2024-
2025 which will be issued by second week of June 2024.

For your registration in July 2024, kindly prepare the following entrance credentials for
submission to the OUR on your designated schedule:

a) UPCAT Acceptance Confirmation Notice – (Downloaded from the UPCA portal)
b) UPLB Notice of Admission – One page printout of the emailed notice
c) Form 138 (First and Second Semester Grade 12 Report Card) – Original Copy
d) Form 137 (Senior High School Transcript of Records) – Original Copy
The link to the letter request for the high school will be sent with the Notice of Admission
e) PSA Birth Certificate (or NSO if the PSA is not yet available)- 2 photocopies with original copy
to be presented for verification
f) Recent and identical ID pictures – 1×1 with white background and name labelled at the bottom,
3 copies
g) Accomplished Student Directory Form (Download copy here)
h) Medical Clearance issued by the University Health Service, UPLB
Additional for non-Filipino citizens only:
i) Passport pages showing name, photo, date and place of birth – Photocopy
j) Study Permit issued by the International Student Relations Office, OVCSA, UPLB

Kindly take note of the tentative dates for registration matters as follows:

24 June 2024 (M). Deadline to accomplish the New Undergraduate Confirmation of Enrollment
Google Form for First Semester 2024-2025.
08 July 2024 (M) to 19 July 2024 (F). Pre-Enrollment Physical/Medical Examination (PPE) for
new first years. Please proceed directly to the UPLB University Health Service on your
assigned date (will be indicated in the registration instructions).
22 July 2024 (M) to 26 July 2024 (F). Advance registration (submission of hard copies of
entrance credentials) for new first years. Please proceed to Windows 2 and 3 of the Office of
the University Registrar, G/F CAS Annex I Building, Pedro R. Sandoval Avenue on the
assigned registration date for your degree program (will be indicated in the registration
instructions).

Only those who have accomplished the Undergraduate Confirmation of Enrollment Google
Form 2024-2025 and submitted their entrance credentials will be issued the UP Mail account
and the Student Academic Information System (SAIS) credentials.

Second/Third week of August 2024. Access your SAIS account to view and download the
schedule of classes for First Semester 2024-2025.

19 August 2024 (M). Start of Classes for First Semester 2024-2025.
