Appeals Requirements for New First Year Admission
Second Semester 2023-2024
Please be informed that the link to the UPLB Appeals Second Semester 2023-2024 Google Form will be
provided from 13 November to 15 November 2023.
Please prepare the following for uploading on the Google Form:
1) Affidavit of non-enrollment for First Semester 2023-2024
2) UPCA Notice (for UPCA qualifiers) or UPG result (non-qualifiers)
3) E-signature
Applicants should not have enrolled in any college/university during First Semester 2023-2024 and
should satisfy the requirements set by the college for a degree program to be considered for admission.
